WEEK 6 OB CASE STUDY BY ZOULIATH

PATIENT HISTORY: IT IS A 23Y OLD PATIENT G5 P5 WHO CAME FROM THE ER FOR PELVIC PAIN MOSTLY ON THE LEFT SIDE AND CRAMPING. NO BLEEDING. THE B HCG WAS POSITIVE. THEY SEND HER TO OUR FLOOR TO CONFIRM PREGNANCY.

In this study, I think I see an intrauterine early gestational sac with a possible subchorionic implantation bleed, and possibly a hemorrhagic corpus luteum cyst in the left ovary.

It does not state whether the pregnancy is within the uterus. Given the "ring of fire" this could be an ectopic pregnancy. There is also a large septated cyst of unknown dimension. There could possibly be a subchorionnic bleed as well as Abby stated.

I agree. Ectopic pregnancy usually occurs within the fallopian tube. There is a risk of rupture and massive hemorrhage. Bleeding is usually a symptom of ectopic pregnancy, but this patient doesn't present with it.

I was able to finally see the images at my home computer, which has new software... try to access it from a computer at school or a computer with fairly new software i guess...

Image 1&2 are somewhat unclear but I believe you are showing the pregnancy and image 3 is measuring the whole left ovary in the sagittal plane which I think is a degenerating hemorrhagic cyst with the corpus luteum of pregnancy. But without seeing it live it is difficult to say.

In her uterus, there is gestational sac, but the image a and 2 does not represent with fetal heart or pole. I might guess it is psedo gestational sac. As going down the next images. we can see another gestational sac on the LT ovary, which implys ectopic pregnancy. I am not quite sure about hemorrhagic corpus leteum cyst

Maybe there is an IUP and also an ectopic pregnancy in left ovary?

i think it is an ectopic pregnancy. ectopic pregnancies are common in 1st trimester pregnancies. The ectopic is at the level of the ovaries.

I wish I could have seen this study in real time, but by just looking at the images, I think the uterine gestational sac was empty and since she was having pain and she had a positive pregnancy test, that is a red flag for the possibility of an ectopic pregnancy. When looking at the left ovary, it looks like ovarian tissue with the large cystic structure that have some septations in the cyst. Then, there is a smaller cystic structure that seems attached as well. The smaller structure looks like it has a thicker wall and when color Doppler is place on the structure the ring of fire sign is important to note.I also think that the smaller structure is not part of the ovary but a dialated fallopian tube it would be nice to see if it was possiable to see a dilated fallopian tube. I think it looks like an ectopic pregnancy. Informative study, I’m interested to find out what it actually was.

In the last image, I see a what seems like a gestational sac adjacent to a complex cystic mass with septations all within the the left ovary. This complex cystic mas could be a corpus luteal cyst. Next to the gestational sac is a subchorionic bleed as mentioned before. There is also a 'ring of fire." All of these images suggest an ectopic pregnancy in the left ovary.

Look like ectopic pregnancy. Big complex cyst at first appears as corpus luteum cyst. And if you look carefully you can see it is next to ovary.
